Information for OGE Filers:

Attention All Confidential Financial Disclosure Filers:

All employees who occupy positions in which the duties and responsibilities are covered under the provisions of 5 CFR 2634 are required to file an annual Confidential Financial Disclosure Report.  The report will cover the period January 1, 2007 to December 31, 2007.  No report is required if the employee performed the duties of the position for less than 61 days during the 12 month period.  Designated filers have all been notified in writing of their requirement to file.

Follow this link to the OGE Form 450 for use during this filing period.  An OGE Form 450-A cannot be filed this year as it is a Presidential election year.

These forms must be received in the BIE Human Resources Office on or before February 15, 2008.  If you feel you have been improperly designated, please discuss this issue with your supervisor.  If necessary, please have your supervisor contact the BIE OGE Filer coordinator, Jacque Stevens, in the BIE Human Resources Office @ 505-563-5317, or by email to jstevens@bia.edu.  The coordinator will work with your supervisor to determine your filing status, and if necessary, have your name removed from the list.  Failure to file by February 15, 2008 may result in administrative sanctions.

If you require additional time to file your report, please contact Ms. Stevens prior to the February 15th deadline.  Employees may request up to a 90 day extension for good cause, i.e., long periods of official travel prior to the due date, significant illness just prior to the due date, and extremely pressing duty assignments.  Extensions must be requested prior to the deadline.

When you complete your form:

  Fill out the form completely.

  Sign in the designated space(s)

  Keep a copy for your records

  Send the original to:
    BIE Human Resources Office
    Attn:  Jacque Stevens
    PO Box 769
    Albuquerque, NM  87103-0769

To protect your privacy, you may place your completed, signed form in a blue "special attention" envelope, marked for Jacque Stevens, Human Resources Specialist, and enclose the blue envelope in an outer envelope addressed to the HR Office. 

Please remember to sign and date your forms.  These forms do not require the signature or approval of your immediate supervisor.  Also, please remember that faxed copies of your financial disclosure forms are not acceptable. 

 

Self-Certification of completed mandatory training
As an OGE-450 Filer, you are mandated to complete annual ethics training before February 15, 2008.  Several options are available to meet this requirement, and are listed below.  Upon completion of your annual training you must certify completion of required training to the ethics coordinator.  The ethics coordinator for BIE is Doris Willie and she can be reached in one of the following ways:

  You may complete the download and read the written materials provided by the Department of Interior as described above.  There is a certificate of completion at the end of the written materials which you may complete, print and sign, then send/fax to the ethics coordinator;

  You may complete the on-line training at http://training.nbc.gov/ethics/ and self-certify completion in writing, then sign and send/fax to the ethics coordinator (here is a sample certification - you can download in MSWord or download in Adobe Acrobat);

  You may read the current Department of Interior Ethics Guide (green) that is available for download here, then complete, sign and send/fax the self-certification to the ethics coordinator;

  You may read a hard copy of the current Department of Interior Ethics Guide available from your Business Manager/Technician, then complete, sign and send/fax in the self-certification form on the back; or

  You may attend a formal Ethics Training, and send/fax in a copy of your completed Standard Form 182 - Request, Authorization, Agreement and Certification of Training.
